Dear Parent,
We would like to welcome your child to the seventh grade for the fall semester. The State of Nebraska requires students entering the seventh grade to have an updated physical on file. Please turn the physical into the office two weeks prior to the first day of school. The athletic physical required to participate in athletics will not suffice. However, the seventh grade physical can be used for athletics.
School immunization rules and regulations also require that each seventh grader have the following immunizations as a condition for attendance in school: 2 doses of MMR or MMRV, given on or after 12 months of age and separated by at least one month; 3 doses of Hepatitis B vaccine; 1 dose of Tdap booster; and 2 doses of varicella (chickenpox) given on or after 12 months of age. Written documentation (including year) of varicella disease from parent, guardian, or health care provider will be accepted. Enclosed are the forms for verification of chickenpox and physical.
